SUTTON OLDER PEOPLES INTENSIVE COMMUNITY SUPPORT TEAM

6 MONTHS FIXED TERM CONTRACT

 The post holder will support and assist the care co-ordinator to deliver intensive treatment and care to people in their own homes, supporting their well being and avoiding admission to hospital. The post holder will be expected to work shifts, including weekends and bank holidays.

For this role, we offer career pathways to becoming an associate nurse. In addition, we offer ongoing training and development whilst in and specific to the post. 

Main duties of the job

  • The post holder will visit people in their own homes, be responsible for their own time management, work autonomously, support the care co-ordinator to deliver the plan of care, monitor medication compliance and encourage same, continually assess for improvement or relapse of condition and feedback to the qualified staff on a daily basis and take part in the Multi Disciplinary team discussions.
  • The post holder will need to be able to work competently and confidently without direct supervision. They will need to have had experience of working with older people. They will need excellent interpersonal and communication skills to facilitate the forming of therapeutic relationships. They will need good observation skills and an ability to act calmly in emergencies and also be able to demonstrate common sense. They will need to be adaptable and flexible.
  • This post requires the post holder to work flexibly across the week as the service is open 365 days a year and the post requires shift working.

Working for our organisation

  • Sutton Older Peoples Intensive community support team is a sub team of the Sutton Older Peoples Services and based at Cheam Resource Centre, London Road, North Cheam. The team is configured of qualified and unqualified staff. The larger team is Multi Disciplinary and comprises of Consultants, junior doctors, Occupational Therapists, Nurses and Psychologists.
  • We are based on the London Road in North Cheam with good transport links to Morden ( London Underground) and to Worcester Park ( Trains to Central London). There are a variety of shops and restaurants in the vicinity.

Detailed job description and main responsibilities

  • To provide practical and emotional support as described within the care plan
  • To provide the link between the care coordinator and the service user
  • To develop a rapport with the service user that facilitates communication
  • To identify changes in the service user’s presentation, mental state and level of functioning and to report back to the qualified staff in a timely and appropriate way
  • To work closely with the rest of the team and the care coordinator

To maintain adequate and contemporary records as required

Education and Qualification

Essential criteria
  • Care Certificate
  • English and Mathematics basic qualifications (eg GCSE at grade C or above) or equivalent
Desirable criteria
  • Level 2/3 Health and Social care qualification / working towards qualification

Experience and Knowledge
